Was the entire world created piece by piece, or by an engine that did it all?

Question Title: Was the entire world created piece by piece, or by an engine that did it all?
Category: Landscape
Question: I have to ask because the world is just so large that it boggles the mind to think that you created every single piece of the world by hand. Was it all done by hand, or did you create an engine or something to design most of the world?

Answer: We designed everything by hand, but the techniques we use are not the same type as the quake style engines. We are able to lay out the world at a very high level very quickly (ie: I want a mountain here, forest over there, etc) and then go back and customize specific areas. Pete laid out the basic height and terrain textures over a several month period while I placed all the trees, bushes, rocks, etc. Dungeons are built using a similiar "high level" technique, which doesn't require our designers to spend time mucking around with polygons and BSP trees.
